Narrative

Worker was repairing a tire for a Cat 777 haul truck. The tire was ""flat on the ground,"" with the tire hand still attached. The worker needed more slack in the air hose and so he ""whipped"" the hose, pulled, but did not realize that it had become tangled in the levers that control the hand. This caused the tire to shift and pin his leg against another tire also ""flat on the ground.""
