Narrative

When checking his ram car batties, he lifted the battery lid (lid was stiff). Lifting the lid, pain was experienced right shoulder. Soreness continued & on 8-27-04. he was treated by Dr. He continued to experience pain & discomfort. He was later diagnosised to have a torn tenden. Surgery was performed on 4-18-05 missing scheduled work days on that date. first day missed: 4-18-05.
