Narrative

Employee was loading a bulk truck. He thought the tank may be empty so he reached outside the handrails and stuck his right hand in a 3""x4"" opening to check for material flow. The air-operated cylinder closed the clam shell gate catching his hand between the cylinder rod clevis and the opening.
