## Narrative

The thickener tank had stopped up. Three people had worked on breaking up the clogged area using a 20' pipe connected to an air line, removing a section of floor grating to do so. The victim went back to start the plant. When he returned he stepped in the hole left by the missing grating and fell into the opening striking his chest on the edge of the floor grating and cut his head.
