Narrative

Employee was using a J-wrench (40 - 50 lbs) to break the lock on the thread between the hammer and the back head on the drill. He was using the supports of the broom as a block to push the wrench against, and was next to the cab when the wrench broke loose under pressure and shot back at him, striking him in the nose. This resulted in a laceration and a fracture to the nose.
