Narrative

The ee stated he was walking around section equipment and stepped in a hole and felt pain in his lower back. The ee went to the dr on 4/12/05 and was supposed to return to work his next shift. Due to the dr not being available he did not return to work but was supposed to return the next shift with a dr's slip to cover his absence. This accident is still under investigation.
