Narrative

Drilling crew was mentally stressed due to mechanical problems. Drill rig (CME850) was in the shop for repairs. Steel HWT casing was piled on the rig above the top of the holding posts. While reaching for tooling, driller knocked casing down and it landed on his foot. Driller did not see it falling because of distraction of poor housekeeping.
