## Narrative

Two employees were getting a 4"" steel pipe from a storage rack. There were several sizes of pipe mixed together on the same rack. One employee lifted the pipe with the forklift. When the other employee rolled a 4"" pipe off the tip of the forks, a 10"" pipe rolled with it. His finger was pinched betweem the two pipes.
