Narrative

EMPLOYEE WAS JACKING AN OVERCAST BEAM UP TO CLEAR THE CONTINUOUS MINER DURING AN EQUIPMENT MOVE. THE JACK BAR SLIPPED OUT OF THE JACK CAUSING HIM TO LOOSE HIS BALANCE AND FALL TO THE MINE FLOOR JAMMING HIS RIGHT SHOULDER
