Narrative

On the surface at Vent Shaft #8, The injured ee and one other ee was attempting to lift one end of metal grating (36 in. x 20 ft). It slipped catching the injured ee right middle finger tip between the grating and the asphalt. Resulting in the amputation of the tip of his right middle finger.(Yes Amputation Tip per call 5/23/05)
