Narrative

Employee states that while he was using a hammer to remove a bearing his right elbow started to hurt. He did not want to go to the doctor. On 5/18/05 he said his elbow was still hurting and now wanted to go to the doctor. He saw the doctor on 5/19/05 and the report was strain or sprain/tendonitis from repetitive movment. No lost time or restricted duty.
