Narrative

EMPLOYEE WAS INSTALLING ROOF BOLT IN TOP AS HE WAS BENDING THE BOLT IT ROTATED CAUSING HIM TO TWIST AND JERK HIS NECK. HE FELT PAIN IN HIS RIGHT SHOULDER AND NECK. LAST DAY AT FULL DUTY WAS 05/16/05
