## Narrative

Employee was using a wrench and extender bar to reverse the screw. Injured and the witness had their body weight pressed against the extender bar when the wrench broke. The two employees fell, their body weight crushed the tip of injured's fourth digit of his right hand between the extender bar and the Cement Cooler.
