Narrative

Employee was getting ready to fuel 60 ton truck when he stepped off loader on the front of the truck. When he stepped down he stepped on uneven ground and sprained his left ankle causing him to fall on his right side, he tried to catch himself with his right hand and twisted his right shoulder and fell on right hip. Pinched siatic nerve.
