## Narrative

Employee was working on a hauler utilizing the man-lift. While in the man-lift basket, employee moved to another location. While moving, the man-lift hit a hole causing the basket to jarr the employee into the handrails hurting his lower back. Employee was prescribed physical therapy starting June 2,2005.
