Unintention roof fall occurred in B Mains, #5 Entry, Between 39 & 40 Block.  Fall measured approximately  10'L X 12'W X 5'H.  Area was mined in December, 1998. Area was previously supported with 5' tension torque roof bolts.  Cover in this area is approximately 250'.
