Narrative

Mechanic was pulling on a hydraulic hose. The connector caught as he yanked on the hose. It came free coming back and smacking him in his mouth breaking his tooth. Cause of this injury was awareness and attitude. Complaince with rules/regs was not a factor, personnel equip. not a factor, miner proficiency a factor, mining equip./systems not a factor.
