Narrative

While employee was on 2 right sect., #6 heading, 9 block a piece of slate 1"" thick x 2' wide x 1 1/2' long fell from between 6' resin bolts with 2""T x 6""W X 12""L hole boards, striking employee on his right hand. Employee hasn't called mine to inform us of his injury. Company has tried several times to reach him.
