## Narrative

On 6/10/05, a mechanic working the afternoon shift was walking from #2 entry to #3 entry when a large piece of rock fell out of mine roof from between permanently installed roof supports, it measured 8' long x 2' wide x 8"" thick. This rock fell out of the mine roof between a roof bolt & the rib, striking him on left lower leg & ankkle, fracturing bones in lower leg.
