Narrative

Injured removing bolt from wheel pin on Grader using a long handle 3/4 drive rachet. The bolt broke & he fell against front push block & hit Rt. elbow. F.A. case at this time. Swelling to elbow on 5-26-05. Sent to company Dr. on 5-27-05. Referred to Orthopedic on 6-13-05. MRI on 6-14-05. Results of MRI on 6-16-05 were 50% tear of Tricep Tendon. Surgery scheduled for end of June.
