## Narrative

Employee did pre-inspection, started Volvo A35,  drove to mud station where truck is loaded.  When stepping out of truck, he stepped from the deck to the bank instead of using ladder off of equipment.  Slipped and missed the bank and hit chest on the top of the bank.
