Narrative

DID NOT RECEIVE MEDICAL INFORMATION UNTIL 7/11/05. Given pain medication and released. Employee was working on the top of the baghouse silos monitor floor. He was measuring bin levels. While walking through to measure Silo 17, he walked into a paddle on the #4 mill line hitting his head and jamming his neck.
