A roof fall occurred above the anchorage horizon. The fall was 21 ft long 14 ft wide and app 8 ft high. On 7-9 at 7:45 supervisor saw the top was sagging and decided to add some additional bolts in the form of 12 cable bolts. The fall was in xcut 1 to 2 at 45 block.
