Narrative

Employee reported: operating a tractor, when unboarding the operator's compartment, stepped on uneven mine floor twisting his left knee and ankle. Continued to experience soreness and stiffness, sought medical treatment on 4-12-05, had surgery on the knee 7-5-05. Became reportable on 7-11-05.
