## Narrative

An unplanned roof fall occurred in the #2 entry of south mains (mmu 001-0) at our No. 5 mine. Fall is located 2 x-cuts inby survey station #74. Fall area is approximately 20' wide, 25' long, 9' to 10' high. Area bolted with 42"" fully grouted resin bolts according to the approved roof control plan. Fall will be cleaned up. Area posted and dangered off.
