Narrative

The employee stated that he had been on the operator's side repairing the dust collection hose. He had returned to the side of the roof bolter to return a screwdriver to his dinner bucket. While standing approx 1' outby the rear at his bucket, a rock from the top fell from between the bolts striking him in the head and forcing head into the roof bolter.
