Narrative

EE was dismounting from the back counterweight step. He had positioned a 6' folding ladder 26"" from the step to assist. As he stepped with his left foot onto the counterweight step, then stepped with his right foot facing the ladder, his left foot slipped through the step opening, which measured 5"" X 11"". He then fell spun around ending upside down, hunging by his leg.
