## Narrative

Employee was using a 1-1/16"" combination wrench on the same size nuts to tighten the clamp rails on a screen shaker.  While applying downward pressure to the wrench, employee said he felt something give way in his left shoulder.  Since then, employee has had difficulty using his left arm and shoulder.
