## Narrative

EE, along with another employee, was replacing a broken Dixon Cam Lock fitting. They had trouble getting the new fitting into the hose and during the process, the cam lock let loose, hitting the employee in the mouth piece of respirator. The employee scheduled a dental appt for 08/16/05, at which time a chip was discovered and capped.
