## Narrative

Employee was throwing cement block through a hole in an existing wall to build another wall on the other side.  Cause of this injury was not making the hole big enough and safety awareness. Compliance with rules/regs not a factor, protective equip ok, miner attitude a factor, mining/equip systems not a factor.
