## Narrative

Employee was standing on grating step attempting to align shear bolt holes on a 9"" dust screw. On third pull of pipe wrench (using excessive force) wrench slipped off the screw section and his body fell back landing on a concrete pad. Struck his left elbow and forehead on concrete.
