Narrative

Employee was dismounting from a bulk truck when he experienced pain in his left shoulder. Doctor diagnosed torn rotator cuff and bicep muscle due to the build up of bone spurs. He was diagnosed on 19 Sept. 2005. At that time he was placed on restricted duty. He is scheduled for surgery to repair the rotator cuff and bicep muscle on 5 Oct. 2005.
