Narrative

Water hose blew off the hose connection bushing. The employee put it back on and tightened it with a 6"" crescent wrench. He turned the water back on and walked away. The hose blew off again and the water hit him in the back, knocking him into the feed box frame. He struck his left eyebrow on the frame and cut it.
