Narrative

Employee was installing a wheel cover while lifting and twisting to get bolt holes lined up, when he felt a slight burning sensation in his left abdomen. Medical treatment was sought on July 6, 05 and he was diagnosed with hernia and released to full duty. Surjury to repair the hernia was conducted on 9/22/05 and he was placed on restricted duty.
