## Narrative

He was tightening a 2"" valve and steel pipe into a 2"" plastic fitting with a pipe wrench on top of a concrete hopper.  A glue joint in the plastic fittings failed.  The pushing action carried the pipe wrench forward and into the 1/8"" steel of the hopper.  The action of the pipe wrench caught the employee's little finger on his left hand.
