## Narrative

Employee was lifting a slab of sandstone using a 36"" crow bar. The crow bar slipped causing the stone to strike his right leg. He declined medical treatment at that time. On 9-26-05 he requested medical treatment for same injury. Physician diagnosis: infected hematoma, employee not to work until further instruction from physician.
