Narrative

Employee was lifting 50lb bags of talc onto a converyor stystem when employee felt pain in groin. On September 7, 2005 employee was evaluated by doctor as having sustained an left abdominal inginial hernia. Employee continued to work until September 20 at which time employee had surgery to repair hernia. On October 3rd the company determined that the incident was work related.
