Narrative

Employee installing fuel tank on 9578 pickup on hoist. Due to design of bed on the truck a straight up lift is not possible. Employee and helper were manually fitting the tank into position which is dfficult. This appears to be a recur of an old injury same body part injured - investigation completed 9/22/05 appears to be work related.
