## Narrative

The injured was roof bolting with a twin head Fletcher, operating left side and was in the process of swinging the head of the roof bolter toward the left rib to install his first bolt in that row, when a piece of head coal approx. 4"" thick x 30"" wide x 24"" long fell from between the bolted area hitting him on his left shoulder.
