Narrative

The employee stated he was in the last open cross cut between the no. 2 & 3 entry, on the company no. 004. continuous miner section. He was in the process of walking by the continuous miner in route to get his scoop, when a piece of draw rock suddenly fell striking him on the head, back and left knee area. The piece of draw rock measured approx. 1'x2.5'x4""thick.
