Narrative

Employee was attempting to close metal sliding railcar load door under palletizer incline. Employee was behind door edge pushing. Door stuck due to dirty rollers. When employee leaned in on door to push harder, door broke loose suddenly causing his feet to slip out behind him. Employee fell onto concrete floor landing on left side. X-Ray diagnosed with broken left hip bone.
