Narrative

Employee was pulling 60"" bottom roller under belt through mud when rope slipped off and he fell on his left shoulder and knee. On 11/2/05 employee had surgery to repair his left knee and began missing work at that time. Shoulder surgery is scheduled for 11/23/05.
