Narrative

Employee has had a history of right knee ailment since 08/24/1998. On 11/12/2005 He began losing time for pain in his right knee with no incident. After consultation with an orthopedic surgeon he was scheduled for surgery on 11/15/2005. This work related illness is due to normal duties as a coal miner over the past 17 years.
