1W#3 Belt - Dust hose came apart and hit employee in the face causing a contusion and laceration to upper center lip and inside of lip.  Laceration was approximately 1/2"" to 3/4"" long requiring 4 sutures, 3 on the outside, 1 on the inside.  MEDICAL TREATMENT ONLY.
