Narrative

Supervisor was pounding out a bit insert on a miner head and a piece of metal flew off and impeded into his leg. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equipment not a factor, miner knowledge could be a factor, mining equipment/systems could be a factor.
