Narrative

An unplanned roof fall occurred in 1 West Room in x-cut #3 to #2, 3 x-cuts inby Survey Station #156. Fall was approximately 4' high, 18' wide, 40' long. Area was bolted with 42"" fully grouted roof bolts according to the approved roof control plan. Area timbered and dangered off.
