Alledged incident was to have occurred while this employee was turning the cowl on the shearer.  He claims a piece of rock came down and hit on top of the shield bouncing it. Cause of this injury was shield was not completely set against the roof. Compliance with rules/regs not a factor, protective equip. not a factor, miner proficiency a factor, mining systems a factor.
