Narrative

Employee stated he has pain in both shoulders from long-term roof bolting. On 11/28/05 employee received an injection in his left shoulder to treat ""frozen"" shoulder. Due to injection, employee missed work on 11/29/05 and returned to full duty on 11/30/05.
