## Narrative

U/g electrician was traveling via Kubota tractor from the electricians shop on the 920 main to the top of 45 decline to access the M165 level in 200S to repair a damaged switch box.  He was traveling north and when he was struck directly on top of the helmet by a piece of loose material, that was aproximately the size of a grapefruit, that had spalled out of the back.
