Narrative

Employee was assisting two fellow employees install rib mesh along the walkway side of conveyor belt entry. As he pushed the wire mash against the rib, a loose piece of rib fell pushing the mesh toward the ee. EE attempted to turn and move away, but his arm/elbow caught on the mesh, resulting in shoulder strain.
